New Patient Registration

As part of the registration process, all new patients aged five and over are offered an appointment with the practice nurse for a new patient health check. This includes checking your blood pressure and urine, measuring your height and weight, giving advice on a healthy lifestyle, and recording any current problems and/or allergies in your medical history.

For each person wishing to register with us, we require either a medical card with the section “What to do if you change your doctor” completed, or alternatively a completed GMS1 form, available from reception. You can now also register with us online.

We also require two forms of identification: one photographic (ideally a passport or driving licence), together with a copy of a utility bill or bank statement.

If you are a carer, or if you have a carer, please remember to let us know when completing your registration form. If you would like a copy of our Carer’s Pack, please ask at Reception.

Change of Personal Details

It is very important that the Practice has up‑to‑date records. We would be very grateful if you could advise reception if you change your address and/or telephone number, or update these details using your online login.

Online 'Pre-Registration' With The Practice

If you wish to pre‑register, click on the link below to open the form. When you have completed all of the details, click on the “Send” button to email your form to us. When you visit the surgery for the first time, you will be asked to sign the form to confirm that the details are correct.

Temporary Registration

If you are ill while away from home, or if you are not registered with a doctor but need to see one, you can receive emergency treatment from the local GP practice for 14 days. After 14 days, you will need to register as a temporary or permanent patient.

You can be registered as a temporary patient for up to three months. This will allow you to be on the local practice list while still remaining a patient of your permanent GP. After three months, you will need to re‑register as a temporary patient or register permanently with that practice.

To register as a temporary patient, simply contact the local practice you wish to use. Practices do not have to accept you as a temporary patient, although they do have an obligation to offer emergency treatment. You cannot register as a temporary patient at a practice in the town or area where you are already registered.
