New Patient Registration
As part of the registration process, all new patients aged five and over are offered an appointment with the practice nurse for a new patient health check. This includes checking your blood pressure and urine, measuring your height and weight, giving advice on a healthy lifestyle, and recording any current problems and/or allergies in your medical history.
For each person wishing to register with us, we require either a medical card with the section “What to do if you change your doctor” completed, or alternatively a completed GMS1 form, available from reception. You can now also register with us online.
We also require two forms of identification: one photographic (ideally a passport or driving licence), together with a copy of a utility bill or bank statement.
If you are a carer, or if you have a carer, please remember to let us know when completing your registration form. If you would like a copy of our Carer’s Pack, please ask at Reception.
Change of Personal Details
It is very important that the Practice has up‑to‑date records. We would be very grateful if you could advise reception if you change your address and/or telephone number, or update these details using your online login.
