New Patient Registration
As part of our registration process, new patients may be offered an appointment with the practice nurse for a new patient health check. This may include checking your blood pressure, reviewing your medical history, discussing any current health concerns and offering general lifestyle advice. These checks are not mandatory for registration but may be recommended depending on your age and health needs.
To register with us, you will need to complete a GMS1 registration form or submit an online registration through our website or the NHS App. Medical cards are no longer required.
We may ask for identification to help us match you to your NHS record, such as a passport, driving licence, utility bill or bank statement. However, ID is not required to register and we will not refuse registration if you are unable to provide it.
If you are a carer, or if you have a carer, please let us know when completing your registration form. If you would like a copy of our Carer’s Pack, please ask at Reception.
Change of Personal Details
It is very important that the Practice has up‑to‑date records. We would be very grateful if you could advise reception if you change your address and/or telephone number, or update these details using your online login.
